Authorization to Photograph

Please select the level of permission to allow your child to be photographed during MHCC activities for the purpose of publicity in the local newspapers, bulletin board displays, newsletters, flyers, and this church's own website.

Photography Authorization *

Full Permission - your child's photo can be used in all communication and social media public platforms, for the purpose of promoting the youth and vibrance of our church. to the wider community. Internal - your child's photo can be used for internal publications such as The Courier, e-news or the FB Members page, but will not be used in newspapers, on public pages or websites. No (please note that your child may be shown in a group situation such as the pageant or a church group activity, but will not be photographed or used close up.)
